By completing this form you are confirming that the learner meets the criteria to book a place on the course / event, and they will complete any pre-course / event activities needed (if advised there are any on the Calendar)

The Adult Social Care Academy will review each request and book a place for the learner if one is available and send a confirmation email to the Manager / Officer making the request and the learner, if an email address is provided for the learner.
